International Missionary Department Forms

These forms are for training purposes only.

To complete and submit the online forms, please complete the following steps:

1. Download the Annual Missionary Roll Call form. This form will be attached to the online Credential Form.
2. Click the Submit Form button on the appropriate form. The form will automatically open in a new browser window.
3. Click Start Filling and enter the information for every field. Enter zeroes or N/A where needed.
4. Once all required fields are completed on the form, click the Preview PDF button. Please review all data and download a copy for your files.
5. Submit payment via PayPal.
6. Local Presidents – Please submit a copy of your completed forms and your Missionary Roll Call spreadsheet to your State/Diocesan President.
