International Missionary Department Forms

To complete and submit the online forms, please complete the following steps:

1. Download the Annual Missionary Roll Call form. This form will be attached to the online Credential Form.
2. Create your Spiritual Report in Microsoft Word. This document will be attached to the Financial Report.
3. Click the Submit Form button on the appropriate form. The form will automatically open in a new browser window.
4. Click Start Filling and enter the information for every field. Enter zeroes or N/A where needed.
5. Once all required fields are completed on the form, click the Preview PDF button. Please review all data and download a copy for your files.
6. Submit payment via PayPal or debit/credit card.
7. Local Presidents – Please email a copy of your completed forms and your Missionary Roll Call spreadsheet to your State/Diocesan President.
